Mental health services provided through Happy Bee Therapy Incorporated are rendered by Rachel Dean, MA, LPC-Associate, who is licensed to practice in the State of Texas under the formal clinical supervision of Samara Orr, MA, LPC-S.

Client Rights & Health Records Requests

Under Texas law (HB 4224 / Texas Health & Safety Code § 181.105), clients have specific rights regarding their healthcare information and consumer protection:

• Requesting Medical Records: Clients have the right to request access to or copies of their health records. To submit a records request, please contact Happy Bee Therapy Incorporated in writing at our practice address or via phone at (832) 979-1588.
• Consumer Complaints: You may also file a consumer protection complaint with the Office of the Texas Attorney General – Consumer Protection Division online at https://www.texasattorneygeneral.gov/consumer-protection.

Texas Behavioral Health Executive Council (BHEC)

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by Texas licensed marriage and family therapists, professional counselors, psychologists, and social workers.

Although not every complaint or dispute involves professional misconduct, the Executive Council can provide information on how to file a complaint.
